An analysis of healthcare mobility between the South and the North illustrates a divided Italy: certain regions are strengthening their capacity to attract patients nationwide, while others are experiencing losses of hundreds of millions of euros resulting from patient outflows. The most recent data show significant differences, reflecting the perceived quality of treatments, waiting times, existent specialty, and the organizational capacity of local regional healthcare systems. While Lombardy, Emilia-Romagna, and Veneto remain appealing, numerous regions in Central and Southern Italy continue to have large negative balances, indicating a significant outflow of patients to areas believed to be more efficient or specialized. The total economic volume approaches €5 billion. Lombardy leads the rankings in terms of positive balances, with a surplus of €580.7 million due to inflows of more than €1.023 billion and outflows of €442 million. Emilia-Romagna has €507.1 million, whereas Veneto has €189.4 million. Positive results were also reported for Tuscany (+54.4 million), Trento (+5.1 million), and Molise (+28.2 million). The worst negative balances were found in Campania, Calabria, Sicily, Puglia, Lazio, and Marche.
